MEDICAL
Hoekstra Transportation offers a BCBS Health Savings Account PPO plan that permits you to obtain care and service from any provider you choose. However, if you select “in-network” providers, your out-of-pocket expenses will be greatly reduced.
PRESCRIPTION DRUG COVERAGE
Hoekstra Transportation offers a “no deductible” prescription drug (Rx) plan. There are several levels of coverage available. For generic drugs, there is no co-pay. For brand-name drugs, the amount you pay varies by the plan you select and the cost of the drug. Minimum and maximum charges may apply.
VISION
Hoekstra Transportation pays your enrollment in the free (for employee only) BCBS plan, with a minimal contribution for dependents. We’ll help keep you and your eyes healthy. You’ll also get a great value on your eye care and wear.
DENTAL
Taking good care of your health includes taking good care of your teeth and gums. Hoekstra Transportation pays for your BCBS dental plan with a minimal contribution for dependent coverage, which provides coverage for dental office visits and procedures, including orthodontia for children.
401(K)
Hoekstra Transportation continues to recognize the efforts you have made to its success and is proud to offer eligible employees a 401(k) plan. The purpose of this plan is to facilitate and assist you in planning for your retirement.
LIFE INSURANCE
Hoekstra Transportation pays for a $25,000 basic life insurance plan for all full-time employees. Additional term life insurance is available for you and your family members.

EMPLOYEE ASSISTANCE PROGRAM
Hoekstra Transportation has contracted with the Employee Assistance Program (EAP) to provide employees and their families with confidential, compassionate, and responsive counseling and referral services (at no cost to you) to offer immediate help in resolving issues, such as marital and family conflict, alcohol and drug abuse, depression, eating disorders, stress and anxiety, grief and loss, and physical abuse.
RIDER PROGRAM
Hoekstra Transportation understands a driver’s need to remain connected with family. So, qualified drivers will get opportunities to take along an immediate family member 12 years and older several times a year. The program is free, and your passengers will ride in comfort just like you on our air ride leather seats.
